Errors uploading images in Campaign form

Using field_attach_form to attach an image field to the campaign form results in errors:

"Invalid argument supplied for foreach() in file_field_widget_submit" seems like a common issue. d.o. threads show up around Field Collection but we're not using Field Collection.

This error prevents you from removing an image from a campaign once you've uploaded it.
