Extending Campaign Admin Lock/Unlock

This will extend issue #39 and PR #499.

Thoughts are to add:

Additional column(s) to store a reason why the campaign is being locked, the date it was locked and possibly the username / contact details of the site admin who locked it.

I also thought perhaps we should email the owner of the campaign / org admin to alert them that it has been locked and why?

I also wonder if we need a mechanism for a campaign organizer to mark a campaign as "fixed" to alert an admin that it is now ready for unlocking (issues have been addressed)?

I'll like to pick this up if this is an extension we need for the current milestones?

I'm not sure you necessarily need a way to mark the campaign as "fixed". Maybe just a way for the campaign organizer to send a message to the site admin.

@mheggeseth Sounds reasonable. Based on the above I propose the requirements be:

• Update UI for Campaign Details to make a locked campaign more obvious and a bit cleaner
• Update UI for Campaign Details to include a link to "Read More / Provide Feedback" (wording to be discussed)
• Clicking on the link will display a modal window with details of when and why the campaign was locked.
• Include a send message form under the lock details (single text area and button) for the org admin to type a message and submit it.
• On submit an email will be sent to the site admin who originally locked the campaign to begin a conversation with the org admin as necessary.

Does that all sound reasonable for now? Is so, I will get started on this.
