Archives ledger item #4 ("Rescope provider-gated RAG safety ideas") to Resolved as obsolete. Single-file, docs-only change to docs/outstanding-issues.md.

The #4 rescope target — a stale ~754-line RAG-safety worktree — is
unrecoverable across every worktree and reachable git ref (pickaxe over all
refs returned zero hits), consistent with the documented branch/worktree
sweeps. Two of its three named ideas already shipped on main: answer-quality
thresholds (rag-extractive-answer.ts + eval-answer-quality) and deep-health
(?deep=1, #585/#587). Only cost-cap preflight was genuinely missing and, per
session decision, is dropped rather than re-filed. Docs-only; no protected
RAG surface touched.

#5 is intentionally left open — another session (#1046) already captured
the same pre-clamp-ordering finding there and kept it as a P3 revisit-note.
#1 stays open, correctly gated on the provider canary + approval.

Resolve the merge conflict against main's concurrent archive edits (keep both the
#15 and #4 resolved rows), and address the review finding: archiving #4
(provider-gated RAG safety ideas) while process-hardening.md still advertised the
same rescope as pending left conflicting durable guidance. Update that bullet to
record the closure (rescue source unrecoverable; thresholds + deep-health already
on main; cost-cap preflight dropped) so /issues and the process-hardening doc agree.
