Ports upstream t3code commits de58ec8 (Add Claude Fable 5 model) and 9d66b10 (Add Claude Sonnet 5 as the default Claude model) onto gits.
Verification: turbo typecheck 14/14, targeted vitest 121/121 (model.test.ts, ClaudeAdapter, ClaudeProvider, ProviderRegistry), oxfmt clean on changed files.
