Custom buttons do not show up on menu bar

Created two custom menu buttons. They never show up on the menu bar. They do show up in the dropdown menu off the ellipsis though. Really would be nice if that could get fixed. Thanks for this extension!

I have this problem too. It seems, at least in my VS Code, that there is only so much space reserve for the menu.
I can only have 8 buttons/icons visible at a time.
When I start disabling the default/standard buttons, my custom buttons become visible.

It's been a while since this new intended behaviour does not allow more than 9 editor actions (8 buttons +1 More Actions 3dots-dropdown-menu icon) in the editor group (they can be moved to the title bar, but their number will still be limited)

Since it's also not possible to reorder them (changing the order in package.json does nothing), when more get added automatically (e.g. clicking into a file enables undo and redo and possibly run or debug buttons) this problem gets worse. Maybe with some of the Customize UI / CSS extensions.

its not working correctly.
indeed you can show up to 8 buttons, still the number icon is not showing when i select some code. it appears only when i right click on the buttons toolbar.
