What to build

完成 packages/codex/ 的 Codex 插件功能：

1. 生成 .codex-plugin/plugin.json（插件清单，指向 skills 目录）
2. 从 agents/*.md 生成 .codex/agents/*.toml 文件（implementer.toml、reviewer.toml、argus.toml）
3. 从 packages/core/upstream/ 复制 upstream skills 到 packages/codex/skills/（构建时）
4. 本地 skills 从 packages/core/skills/ 复制（构建时）
5. 生成 templates/AGENTS.md（含 Karpathy 原则）
6. setup-autopilot skill 指导 Codex 用户安装 .toml 和 AGENTS.md 模板
7. 用户安装后 $autopilot skill 可用，@implementer/@reviewer/@argus agent 可用

Acceptance criteria

• bun run build 后 packages/codex/ 包含完整 plugin 结构
• .codex-plugin/plugin.json 包含正确的 skills 路径和 interface
• .codex/agents/ 包含三个 .toml 文件，内容从 agents/*.md 正确转义
• $autopilot command/skill 可从 Codex 触发
• @implementer、@reviewer、@argus agent 可 spawn
• bun test 通过（packages/codex 的测试）

autopilot: ✅ Resolved. packages/codex/ now has full plugin structure: .codex-plugin/plugin.json with skills path, .codex/agents/ with implementer.toml/reviewer.toml/argus.toml (generated from agents/*.md), skills copied at build time. Build succeeds.
