This PR adds a pull request template and makes our contributing guidelines more strict (every PR needs a linked issue now).
I've also overhauled our docs a bit. The quickstart is now more informative and explains to user how they can setup Semble (MCP, AGENTS.md, or sub-agents). The AGENTS.md section was mostly overlapping with the separate section so that one is gone. MCP and sub-agents sections are still there since they have info for other harnesses. Also made some minor other edits to make the flow nicer.
