Problem
Replacing goal A with goal B while A still had an active user turn or automatic continuation allowed late asynchronous work from A to resolve against the current goal. As a result, A's usage, turn accounting, terminal outcome, cancellation or failure handling, and continuation scheduling could mutate, charge, pause, block, or drive B.
The root cause was that goal lifecycle hooks tracked only turn IDs and read the current goal when callbacks settled, without preserving the goal identity that owned the turn.
What changed
The change stays within the Agent-scoped goal service. It does not change persistence records, wire contracts, public APIs, or the loop scheduler.
