Form Intakes - full, half and third width are not working

Description

On additional entities, the field width is always full, no matter how it is configured (half or third).

Environment

1. OpenCTI 6.9.18

Reproducible Steps

Steps to create the smallest reproducible scenario:

1. Create Form
2. Create additional entity
3. Configure subsequent field in half of third width
4. Update and open form.

Expected Output

Additional entity subsequent field to be displayed in half or third width.

Actual Output

Additional entity subsequent field is displayed in full width.
