docs: refresh RUNBOOK cache section for P4-2 - #27

The P4-1 scope note still said index_weight/suspend_d/namechange/stk_limit/ stock_basic were uncached until P4-2/P4-3 — now stale since P4-2 shipped. Fix the scope note (only daily_basic/fina_indicator/index_member_all remain, P4-3), note the run-log cache line now carries all cached endpoints, and add a Phase 4-2 section (semantics, the combined stats line, cache layout, real-smoke).

Docs-only follow-up to PR #25 (P4-2). The P4-1 scope note in RUNBOOK.md still listed
index_weight/suspend_d/namechange/stk_limit/stock_basicas uncached-until-P4-2/P4-3, which is now stale. Corrects the scope note (onlydaily_basic/fina_indicator/index_member_allremain for P4-3), notes the run-log cache line now carries all cached endpoints, and adds a Phase 4-2 section (preserved semantics, combined stats line, cache layout, real-smoke). No code change.
