chat_utils.pyfile address cases where only the first occurrence of a document is updated, while the rest remain unchanged.answer.pyfile ensure that references with "DOCUMENT <no.>" are correctly added to citations. To achieve this, occurrences of "DOCUMENT <no.>" are replaced with "[[no.]]".
