Multiple publisher/collection records per CSV file

The bulkloader should support CSV files with publisher and collection columns so that a single CSV file can contain records from multiple publishers and collections.

Here's what we're thinking it terms of the JSON payload for supporting this:

If a CSV file doesn't have publisher and collection columns, the assumption is that the file contains records for a single publisher/collection which would have to be specified via command line parameters.
