Fixes ADA-115

Summary

  • trim t3_spawn_subagent and t3_thread_start to prompt/model/title plus optional directory/branch/reasoningEffort
  • default Codex reasoning effort to xhigh while allowing an explicit override
  • replace check/list with t3_subagents, remove t3_wait_subagent, and retain peer-only rolling-upgrade compatibility
  • update conformance coverage, wake guidance, and obsolete wait-specific tests/docs

Verification

  • factory gate passed for both commits, including after rebase onto origin/main
  • 164 focused MCP/conformance/Claude adapter tests passed
  • vp check passed
  • vp run typecheck passed
