[FEATURE]:allow /disconnect from cli

Describe the enhancement you want to request

after you connect to a provider, sometimes mistakenly (for example connected to zen instead of go) there is no way to disconnect without getting out of opencode, editing files and hoping it wasn't cached. in my case it tries spawning gpt nano agents as i "have" zen, which fail

This issue might be a duplicate of existing issues. Please check:

• Cannot disconnect from provider #10494: Cannot disconnect from provider (closed bug report about the same inability to disconnect from a provider)
• Google config provider cannot be disconnected (optimistic update only) #12384: Google config provider cannot be disconnected (closed bug about disconnecting providers)

These closed issues addressed similar problems. If the fix didn't fully resolve the use case you're describing (e.g., no CLI command for intentional disconnect/logout), it may be worth referencing them for context.

These two are for the app/web not cli. The cli doesn't have a disconnect option at all. While editing the files is an option, i think it would be much cleaner to have /disconnect
