Feature request: ability to split output into multiple files

Option to create an index file with hyperlinks to each module would be nifty.

Then the ability to go a level deeper so that each class and function gets its own page would be next level (literally).

Sounds good. For page names, should everything be kept flat?

For example, if we had:

• foo.bar.Baz
• foo.bar
• foo

Should we end up with:

foo.bar.Baz.md
foo.bar.md
foo.md
DOCUMENTATION.md

Or:

foo/bar/Baz.md
foo/bar.md
foo.md
DOCUMENTATION.md

I prefer the former (all flat).
