chore(app-server): update Codex protocol to 0.122

Summary

• Regenerate app-server protocol types from Codex 0.122.0 and harden protocol codegen/postprocess tooling.
• Add 0.122 app-server wrapper fields, command exec controls, hook/approval notification handling, and permissions approval parsing.
• Pin bundled Codex binary fallback to rust-v0.122.0 for CI/release workflows.
