Remove support for ~/.appland file.
Recognize APPMAP_* env vars.
APPLAND_* env vars are deprecated but will still work.
Assign triage status to findings. Each finding is identified by hash_v2.
Finding status can be:

* deferred
* as-designed
* active

Findings status is saved to file appmap-scanner-state.yml.
When scanning, filter out findings that are deferred or as-designed.
Command flags can be used to emit findings that would be filtered out by default.

Require AppMap API key to be available in order to scan.
