Add opt-in instrumentation for `fs`

Should only be created if requireParentSpan is true. Maybe we also filter out any fs spans that are below a certain length to make sure they don't get spammy.

Note: Currently fs is not instrumented with diagnostics channel. We perhaps want to upstream contribute to node to add diagnostics channels to fs before moving on with this.
