This removes
Canada/East-SaskatchewanandUS/Pacific-NewfromKnownIanaTimeZoneNames. They will still work in conversion.It also removes
Kamchatka Standard TimeandMid-Atlantic Standard TimefromKnownWindowsTimeZoneIds. They will still work in conversion.Fixes #89
