Configure location of server

This extension needs to connect to https://github.com/mhewett/go-example-server, which is presumed to be running on localhost. It would be nice if the extension exposed a configuration variable that allowed me to point it someone else (e.g. if I am hosting the server somewhere other than localhost).

I set up the configuration variables for protocol, host, and port in the extension code but I don't know how to make them appear in the configuration window.

package.json needs to declare the configuration you want to declare like this:

Then you can access the config like this:
