Populate survey links with variables

When I spec'ed our survey prompting logic, I didn't take into account that we should populate the variables in the survey link (like VS Code does).

For example from VS Code repo:

See the part where they append platform, product version and machine id to the survey link.

• Change should go here: src\client\activation\extensionSurvey.ts
• Extend the existing URL to contain following variable:
  • machine id
  • vscode version
  • extension version
  • platform (windows, linux, mac )

It should look like:
https://aka.ms/AA5rjx5?o={platform}&v={vscodeversion}&e={extensionversion}&m={machineid}
