Always use debug adapter descriptor

We currently have debug adapter descriptor behind a experiment flag (DebugAdapterFactory - control, DebugAdapterFactory - experiment). With this work item we should remove the experiments flag and always use debug adapter descriptor.

Let's still leave the experiment on experiments.json file

1. DebugAdapterDescriptorFactory
2. DebugConfigurationResolver
3. api.ts (test with azure functions extension)
4. Under debugAdapter folder, there's a hooks directory. There are multiple usages of this experiment there where it should be removed
5. Update the tests for all the classes that were touched
6. Tests everything manually (check that nothing broke) but opting in/out the experiments
