Implement click-to-focus jump-back for KDE Plasma notifications using notify-send actions and KWin/kdotool focus routing.\n\n- Add Linux action-token parsing for notify-send (focus-terminal)\n- Wire notification action click to focusTerminal only on KDE+kdotool\n- Add startup window-id capture for deterministic jump-back\n- Keep manual OPENCODE_NOTIFIER_WINDOW_ID override\n- Remove temporary debug logging paths\n- Document KDE behavior, limitations, and scope in README\n\nCo-authored-by: Copilot <223556219+Copilot@users.noreply.github.com>
