This pull request fixes duplicated and reordered nested handoff history.
The default mapper now emits chronological summary segments around lossless message items, ensuring each logical item appears exactly once while preserving the original message and handoff ordering. Custom mapper output remains exact, session history retains complete run items, and no RunState schema changes are required.
