feat(app): add help popovers and deep-links to permission rows#9

Each permission row shows a (?) icon that opens a popover with step-by-step instructions. If the permission is missing, a button opens the exact System Settings page. Replaces the generic "Open Privacy & Security" button with a per-row Refresh button.
