Opening documents with many tracked changes (800+) was very slow. The root cause was the comment creation loop during import - it dispatched a ProseMirror transaction for each tracked change, and each one triggered a full document scan. For a document with 872 tracked changes, that meant ~824 dispatches and ~824 full doc scans.
This PR fixes it by:
Tested with a real-world document. Load time dropped from 5+ seconds to ~2.5 seconds.
